Abstract: A 57-year-old patient with a fever, focal erythema and progressive painful paralysis of the lower limbs A 57-year-old patient is admitted to the hospital with progressive paralysis of the lower limbs since three days. He has a fever and reports painful nodules with erythema on both arms and legs. Because of a high inflammatory response and no immediate focus of infection, the initial differential diagnosis included an auto-immune reaction or vasculitis. One day later however, methicillin-sensitive Staphylococcus aureus (MSSA) is cultured from blood cultures. Subsequent investigation leads to the diagnosis of disseminated Staphylococcus aureus bacteremia (SAB) with multiple muscular foci, for which adequate treatment is administered. The clinical presentation, epidemiology, diagnosis and treatment of SAB are discussed.
