He has a fever and reports painful nodules with erythema on both arms and legs. Because of a high inflammatory response and no immediate focus of infection, the initial differential diagnosis included an auto-immune reaction or vasculitis. One day later however, methicillin-sensitive Staphylococcus aureus (MSSA) is cultured from blood cultures. Subsequent investigation leads to the diagnosis of disseminated Staphylococcus aureus bacteremia (SAB) with multiple muscular foci, for which adequate treatment is administered.
