Issue Date: | 2021 | Abstract: | With an estimated 19.3 million new cases in 2020, resulting in over 10.0 million
related deaths, cancer became an inevitable barrier to further increasing our wellbeing and life expectancy. Although traditional clinical tumor treatments, e.g.
surgery, chemotherapy, and radiotherapy, are resulting in increasing survivability,
disadvantages are encountered due to poor efficiencies and severe toxic side
effects to healthy tissue. A variety of alternative techniques emerged throughout
the years, in which the non-invasive photodynamic therapy plays an important
role. This therapeutic approach makes use of three, individually harmless
components: light, oxygen, and a light-activated molecule called a
photosensitizer. Once located at the cancer tissue site, the photosensitizer is
excited using light of a suitable wavelength. As a result, the photosensitizer will
transform molecular oxygen, present in the cells, into singlet oxygen. This reactive
oxygen species will interact directly with different cell components, vasculature
shutdown will take place, and immune responses will be activated. All these
coinciding processes will contribute to the successful eradication of the cancer
cells.
Thanks to the precise irradiation and short half-life of singlet oxygen,
photodynamic therapy enables the highly selective removal of cancerous cells
while avoiding systemic toxicity and side effects caused by damage of surrounding
healthy tissue. The technique is approved by the United States Food and Drug
Administration and is already applied around the world for the treatment of a
broad scope of tumors, but also for treatment of non-neoplastic dermatological
and ophthalmic diseases, rheumatoid arthritis, inactivation of viruses, and to treat
bacterial and fungal infections. However, for a real breakthrough of this
therapeutic process, some drawbacks related to the photosensitizer should be
cleared. First, well-established photosensitizers are absorbing blue to green light. These high-energy wavelengths are easily scattered and absorbed while passing
through the patients tissue. Hence, red to near-infrared activation is desired.
Second, in order to obtain high singlet oxygen production, the photosensitizer is
usually decorated with bromine, iodine or metal complexes. Unfortunately, this
straightforward technique often leads to increased dark cytotoxicity, referring to
the unwanted cell-death in the absence of an activating light beam. Third, these
heavy atoms also strongly quench the fluorescence of the used compounds as
both processes are competing decay pathways from the same excited state.
Nevertheless, the emission of light from the photosensitizers could be beneficial
for outlining tumor boundaries, monitoring drug distribution, and providing realtime follow-up of the therapeutic process. A fourth point of improvement could be
the administration of the drug. As most photosensitizers are water-insoluble, their
ability to generate reactive oxygen species is quenched when introduced in cellular
media.
Luckily, for all problems a solution can be found and these are described in this
thesis. Strongly fluorescent perylene diimide and boron dipyrromethene dyes were
used as a starting point for the development of dual-functioning photosensitizers.
Structural modifications were employed in order to shift their absorption and
emission into the phototherapeutic region. The electron deficient molecular cores
were covalently bound with an electron rich moiety in order to create a donoracceptor system. With this molecular design, singlet oxygen formation was
established without the aid of halogens or other heavy atoms. This approach also
retains sufficient amount of fluorescence, opening pathways for potential use in
image-guided photodynamic therapy. The next step toward suitable
administration was also established by incorporating the photosensitizers into
biodegradable and biocompatible polymeric nanoparticles. Validation of the
efficacy is ongoing at this moment through tests in vitro in cells and in vivo in
mice. Met naar schatting 19,3 miljoen nieuwe vaststellingen in 2020, resulterend in meer dan 10 miljoen gerelateerde overlijdens, blijft kanker een ontegensprekelijke barrière voor de verdere verbetering van ons welzijn en levensverwachting. Hoewel traditionele kankerbehandelingen, zoals chirurgische verwijdering, chemotherapie en radiotherapie, resulteren in toenemende overlevingskansen, zijn er nog steeds nadelen toe te schrijven aan de beperkte efficiëntie en ernstige, schadelijke bijwerkingen ten opzichte van gezond weefsel. Binnen de doorheen de jaren ontwikkelde alternatieve technieken neemt noninvasieve fotodynamische therapie een belangrijke plaats in. Deze methode maakt gebruik van drie individueel onschadelijke componenten, nl. licht, zuurstof en een door licht geactiveerde molecule of fotosensitizer. Eénmaal in het kankerweefsel wordt deze laatste aangeslagen door licht van een geschikte golflengte. Hierdoor zal de fotosensitizer moleculaire zuurstof, aanwezig in de cellen, omzetten in singlet-zuurstof. Dit zeer reactief deeltje interageert onmiddellijk met verschillende celcomponenten, het zal zorgen voor het vernauwen van de bloedvaten en immuunresponses zullen worden geactiveerd. Al deze processen zullen samen zorgen voor de succesvolle vernietiging van de kankercellen. Dankzij een precieze belichting en de korte levensduur van singlet-zuurstof, maakt fotodynamische therapie het mogelijk om een tumor zeer selectief te verwijderen zonder systemische toxiciteit of schade aan omringend, gezond weefsel. De techniek is goedgekeurd door de Food and Drug Administration en wordt reeds wereldwijd toegepast voor de behandeling van verscheidene tumors, maar ook voor niet-neoplastische dermatologische en oogheelkundige ziekten, artritis, inactivatie van virussen en de behandeling van bacteriële en schimmelinfecties. Voor een echte doorbraak van deze therapie zullen enkele fotosensitizer-gerelateerde nadelen echter nog moeten worden verholpen. Ten eerste absorberen de bekendste fotosensitizers blauw tot groen licht. Deze hoogenergetische golflengtes worden echter makkelijk verstrooid en geabsorbeerd door omliggend weefsel. De voorkeur wordt daarom gegeven aan activatie door rood tot nabij-infrarood licht. Ten tweede wordt de mogelijkheid van fotosensitizers om singlet-zuurstof te produceren vaak bekomen door het molecule te decoreren met broom, jood of metaalcomplexen. Ondanks deze eenvoudige tactiek leidt deze manier van werken vaak tot toenemende cytotoxiciteit in de afwezigheid van licht. Ten derde zorgen deze zware atomen voor een sterke vermindering van de fluorescentie, aangezien beide vervalprocessen in competitie met elkaar staan. Desondanks zou emissie van licht van de fotosensitizer zelf voordelig kunnen zijn in het visualiseren van de tumor, het verifiëren van de verdeling van de fotosensitizer in de cellen en een directe opvolging van het therapeutisch proces. Een laatste verbeterpunt heeft betrekking tot het toedienen van de fotosensitizers. Aangezien de meeste onoplosbaar zijn in water, worden hun capaciteiten tot het vormen van singlet-zuurstof sterk gereduceerd wanneer ze ingebracht worden in een waterige celomgeving. Gelukkig kunnen voor alle problemen oplossingen worden bedacht en deze worden besproken en toegepast in deze thesis. Sterk fluorescerende peryleendiimides en boordipyrrometheen-kleurstoffen werden gebruikt als startpunt voor de ontwikkeling van een optimale dubbel-functionerende fotosensitizer. Structurele aanpassingen werden geïmplementeerd om de absorptie en emissie te verschuiven naar het fototherapeutische werkgebied. De elektronenarme kern werd covalent gebonden aan een elektronenrijke eenheid om zo een donoracceptor systeem te creëren. Via dit design kan singlet-zuurstof worden opgewekt zonder de hulp van halogenen of andere zware atomen. Deze methode behoudt ook voldoende fluorescentie voor een mogelijk gebruik in beeldgestuurde fotodynamische therapie. De volgende stap richting geschikte toepassingen werd ook beschreven door het inbrengen van de fotosensitizers in biocompatibele en bio-afbreekbare polymere nanodeeltjes. |
