Issue Date: | 2021 | Abstract: | There is no consensus yet regarding the best surgical treatment of the painful
degenerative TM joint at the base of the thumb. Resection of the trapezium bone
with or without an additional tendon procedure is most often proposed. This
contrasts with the treatment at most other articulations of the human skeleton.
‘Resection arthroplasty’ has generally been abandoned because joint replacement
surgery with implantation of a prosthesis typically produces better outcomes. Ballin-socket prostheses for the basal joint of the thumb are available for almost 50
years. These implants have been perfected following the experience of hand
surgeons, mostly in France. Today, ball-in-socket prostheses are capable of
achieving excellent long-term clinical results in the hands of highly specialized
hand surgeons. In less experienced hands, many complications are possible. The
learning curve of TM TJA is high. The goal of this thesis was to standardize and
quantify some important steps of this operation. Cup placement in the trapezium
is the most critical part of the procedure. Central placement optimizes bone
contact. The cup must be aligned with the centre of TM joint motion to prevent
dislocation due to internal impingement. This is achieved by placing the cup
parallel with the proximal articular surface of the trapezium. Correct cup position
can be checked during surgery with fluoroscopy. The associated radiation risk is
negligible. The stem can be inserted in the thumb metacarpal without cortical
contact, as long as stable press-fit fixation is achieved. This lowers the risk of
metacarpal fracture during surgery, prevents postoperative bone weakening and
facilitates eventual removal of the stem. Correct management of the soft tissue
envelope around the TM joint is also important. It has been recommended to
perform a complete capsuloligamentous release to facilitate exposure of the
operative field and to prevent ligament tethering. This will also increase TM joint
motion, but not beyond the limits of most TM prosthesis. In addition, the risk of
dislocation when the thumb is being pulled is raised, at least during the first weeks
after surgery before scarring has occurred. This can be compensated by using a
longer prosthesis neck at the expense of thumb lengthening. Further research is
needed to investigate if it is possible to retain or suture specific ligaments to
prevent this. The latest generation of double mobility implants provides increased
stability and a lower chance of dislocation due to a larger diameter prosthetic head
and a polyethylene liner that allows more motion. High quality clinical follow-up
studies and implant registries will be mandatory to see if this is really the next
step forward in the future of TM joint replacement. Ball-in-socket TM prosthesis
can almost mimic the results of the most successful implant in orthopedic surgery,
the total hip arthroplasty. Some complications like bony overgrowth around TM
implants remain a concern that is still not well understood. The development of a
truly anatomical TM joint replacement should remain a part of future research
efforts. Er bestaat nog geen eensgezindheid over de beste chirurgische behandeling voor een pijnlijk versleten duimbasisgewricht. De oudste en tot op heden meest gebruikte operatietechniek bestaat uit het verwijderen van het trapezium been. Dit soort ingreep waarbij een deel van het pijnlijke gewricht wordt verwijderd, wordt ‘resectie arthroplastiek’ genoemd. Ter hoogte van de meeste andere gewrichten van het menselijk lichaam werd deze behandeling ondertussen verlaten. Het vervangen van een versleten gewricht door een kunstgewricht of prothese levert daar immers betere resultaten op. De eerste ervaring met gewrichtsvervanging ter hoogte van de duimbasis dateert van bijna 50 jaar geleden. Dankzij de inzet van vooral Franse handchirurgen werd de duimbasisprothese steeds betrouwbaarder. Dit leidde tot de ontwikkeling van een kogel-in-pan prothese die uitstekende resultaten op lange termijn kan voorleggen. In weinig ervaren handen zijn echter veel complicaties mogelijk omdat de leercurve voor deze ingreep hoog is. Het doel van deze thesis was om belangrijke stappen van de operatietechniek te standaardiseren en te kwantificeren. De meest kritische stap is de correcte plaatsing van de prothesepan in het trapeziumbot. De pan moet centraal worden geplaatst én evenwijdig met het proximale gewrichtsoppervlak van het trapezium. Dit orienteert de pan in lijn met het bewegingscentrum van het gewricht, wat de kans op ontwrichting van de prothese tijdens het bewegen van de duim minimaliseert. De juiste plaatsing kan tijdens de operatie met radioscopie worden gecontroleerd. Het stralingsrisico van deze nieuwe techniek is verwaarloosbaar. Ook werd bewezen dat de prothesesteel in het metacarpaalbot van de duim zonder hard botcontact kan worden geplaatst, op voorwaarde dat de fixatie in het sponsachtige bot stabiel is. Dit verlaagt het risico op een breuk van het metacarpaalbot tijdens het inbrengen van de steel. Bovendien voorkomt het dat op termijn botverzwakking rond de steel zou optreden en vergemakkelijkt het verwijderen van de steel moest dit ooit nodig zijn. Vroeger werd aangeraden om het gewrichtskapsel tijdens de operatie volledig los te maken. Dit vereenvoudigt de toegang tot het gewricht en voorkomt na de operatie dat het kapsel verkeerd zou aanspannen tijdens het bewegen van de duim. Het gevolg is wel dat de beweeglijkheid van het duimbasisgewricht vergroot, maar de mobiliteitsgrenzen van de moderne duimprothesen worden normaal niet overschreden. De keerzijde is dat de prothese zonder omgevend kapsel gemakkelijker kan ontwrichten wanneer aan de duim wordt getrokken. Dit risico is het grootst tijdens de eerste weken na de ingreep wanneer er nog geen littekenvorming is opgetreden. Het plaatsen van een langere prothesenek kan dit opvangen, ten koste van een verlenging van de duim. Verder onderzoek is nodig om aan te tonen of een deel van de ligamenten van het duimbasis gewricht kan worden gespaard of kan worden hersteld aan het einde van de operatie. Enkele jaren geleden werd een nieuwe generatie duimbasis prothesen geïntroduceerd. Deze hebben een grotere prothesekop met dubbele mobiliteit. Dit laat meer beweging toe en vermindert de kans op luxatie. Lange termijn opvolgstudies en prothese registratie zijn nodig om aan te tonen of dit echt een stap voorwaarts betekent. Een perfect geplaatste kogel-in-pan duimbasis prothese kan de resultaten van het meest succesvolle implantaat in de orthopedische chirurgie, de totale heupprothese, nagenoeg evenaren. Sommige complicaties zoals beenderige overgroei rond prothesecomponenten ter hoogte van de duimbasis blijven een reden van bezorgheid omdat de oorzaak nog niet duidelijk is. Daarom moet de ontwikkeling van een anatomische ‘resurfacing’ duimbasis prothese deel uitmaken van toekomstig wetenschappelijk onderzoek. |
