Issue Date: | 2022 | Abstract: | Spinal cord injury (SCI) is a severe chronic condition that can result in paralysis,
loss of sensation, incontinence, and neuropathic pain. Consequently, it has a
significant impact on the quality of life and life expectancy of the patients and
leads to a substantial economic burden on society. Over the past decades, there
was substantial progress in the SCI field, yet no curative therapy is available. This
highlights the need to identify new therapeutic targets and interventions.
The pathology of SCI is characterized by the primary traumatic injury, followed
by secondary injury processes that induce further tissue damage and impair
recovery. One of the most critical characteristics of this phase is the excessive
inflammatory response, leading to the activation and infiltration of various
immune cells. Both T cells and monocyte-derived macrophages (MФs) play a
critical role in the inflammatory reaction after SCI. Our study aimed to modulate
these cells and dampen the inflammation to stimulate recovery. We used a T cut
hemisection mouse model in which both dorsal horns, and dorsal and ventral
funiculi are cut. As a result, hind limb paralysis and a well-defined lesion are
obtained.
At first, the role of L-arginine in the pathophysiology of SCI was investigated by
means of its systemic depletion via the repeated intraperitoneal administration of
recombinant human arginase-1 (rArg-I). The data presented in this thesis
demonstrate that rArg-I successfully improved locomotor recovery and reduced
the inflammatory response after SCI (Chapter 3). T cell proliferation was halted
when rArg-I was added. Furthermore, the diminished inflammation was associated
with a decrease in nitric oxide production and neuronal cell apoptosis.
In the second part of this study, it was investigated whether Arg-1 needs to be
injected systemically and if a local Arg-1 overexpression is sufficient to obtain the
aforementioned results. Unfortunately, both the intraspinal injection of Arg-1
overexpressing mesenchymal stem cells as well as MФs failed to reduce functional
impairment (Chapter 4). The second strategy to limit inflammation after SCI, was treatment with
recombinant tissue inhibitor of matrix metalloproteinases 1 (rTIMP-1). This
anti-apoptotic protein is an endogenous inhibitor of matrix metalloproteinases
(MMPs), responsible for the disruption of the blood-brain barrier. Recombinant
TIMP-1 treatment had beneficial effects after traumatic brain injury, yet its effect
on SCI remained to be clarified. Therefore, in Chapter 5, the effect of TIMP-1
modulation after spinal cord trauma was studied. TIMP-1 knockout led to a
worsened outcome after spinal cord hemisection in mice. Nonetheless, the
underlying mechanisms for these negative effects still need to be unraveled.
Moreover, rTIMP-1 treatment did not affect functional recovery.
Taken together, the results described in this thesis show that L-arginine
modulation can be applied as a treatment to restore function in mice after a T-cut
hemisection SCI. Further research is needed to determine whether TIMP-1
modulation holds promise as an immunomodulating approach for SCI. Ruggenmergschade is een ernstige chronische aandoening dat verlamming, verlies van sensatie, incontinentie en neuropatische pijnen tot gevolg kan hebben. Bijgevolg heeft een ruggenmergletsel een enorme impact op de levenskwaliteit en levensverwachting, maar heeft het ook een grote economische impact op de samenleving. Ondanks de medische vooruitgang de laatste decennia, is er tot op heden geen behandeling die het volledige herstel van een patiënt met ruggenmergschade kan garanderen. Dit benadrukt het belang van onderzoek naar het identificeren van nieuwe therapeutische targets en de ontwikkeling van nieuwe interventies. Het ziektebeeld is gekenmerkt door een primaire schade aan het ruggenmerg gevolgd door secundaire processen die verdere schade aan het ruggenmerg berokkenen en nadelig zijn voor het herstel van de patiënt. Een van de meest kritische onderdelen hiervan is de excessieve ontstekingsreactie die leidt tot de activatie en infiltratie van verschillende immuun cellen. T cellen en macrofagen (MФs) spelen een belangrijke rol in deze ontstekingsrespons. Het doel van ons onderzoek was deze cellen te moduleren en de ontstekingsreactie te onderdrukken om zodoende herstel te stimuleren. Om dit te onderzoeken maakten we gebruik van een T cut hemisectie muis model waarbij beide dorsale hoorns, en dorsale en ventrale funiculi zijn doorgesneden. In deze muizen is bijgevolg verlamming van de achterpoten en een goed gedefinieerde laesie waar te nemen. Initieel werd de rol van L-arginine in ruggenmergschade onderzocht door middel van zijn systemische depletie via herhaaldelijke intra peritoneale toediening van recombinant humaan arginase-1 (rArg-I). De data in deze thesis geeft aan dat rArg-I succesvol functioneel herstel en een vermindering van de ontstekingsrespons na ruggenmergschade teweeg brengt (Hoofdstuk 3). T cel deling was onderbroken na het toedienen van rArg-I. Eveneens was de onderdrukte ontstekingsreactie gekenmerkt door een daling van stikstof oxide en neuronale cel sterfte. In een tweede deel van deze studie werd er onderzocht of Arg-1 systemisch geïnjecteerd moest worden, of of een lokale overexpressie ook voldoende was om de reeds vermelde resultaten te bekomen. Helaas hadden zowel de intraspinale injectie van Arg-1 overexpresserende mesenchymale stamcellen als MФs geen effect op het functioneel herstel (Hoofdstuk 4). Een tweede strategie om de inflammatie na ruggenmergschade te limiteren was door de behandeling met recombinant ‘weefsel inhibitor van matrix metalloproteinasen 1’ (rTIMP-1). Dit anti-apoptotisch eiwit is een endogene inhibitor van matrix metalloproteinasen (MMPs), verantwoordelijk voor de onderbreking van de bloed-hersen barrière. Recombinant TIMP-1 initieerde reeds voordelige effecten na traumatische hersenletsels, echter is het effect na ruggenmergschade nog niet beschreven. In Hoofdstuk 5 werd daarom het effect van TIMP-1 modulatie na ruggenmergschade bestudeerd. TIMP-1 knockout verhinderde functioneel herstel na een dwarslaesie in muizen. Echter had toediening van rTIMP-1 geen effect op het herstel van de motoriek. Desalniettemin is het noodzakelijk de onderliggende mechanismen te ontrafelen. Samengevat, de resultaten beschreven in deze thesis tonen aan dat L-arginine modulatie zorgt voor herstel van functie en kan worden gezien als een behandeling voor een dwarslaesie in muizen. Verder onderzoek is nodig om te bepalen of TIMP1 modulatie een veelbelovende immuun-modulerende therapie is voor de behandeling van ruggenmergletsels. |
