Issue Date: | 2022 | Abstract: | In 2020 werden wereldwijd 19 292 789 nieuwe kankerdiagnoses vastgesteld.
Bijna 60% van deze patiënten werd behandeld met chemotherapie. Het doel van
chemotherapie is om de celdeling te remmen en daardoor uitzaaiingen en
metastase te voorkomen. Chemotherapie heeft echter niet enkel invloed op
kankercellen en beschadigt ook andere cellen in het lichaam. Dit resulteert in een
reeks nevenwerkingen die een grote invloed hebben op de levenskwaliteit van de
patiënt. Chemotherapie-geïnduceerde perifere neuropathie (CIPN) wordt
gekenmerkt door symptomen zoals gevoelloosheid, pijn en een branderig gevoel
in de distale ledematen. Deze aandoening treft bijna 70% van de patiënten
behandelt met een neurotoxische chemotherapie. Tot op heden is er onvoldoende
wetenschappelijk bewijs voor een effectieve preventieve of therapeutische
behandeling voor deze aandoening. In ernstige gevallen van CIPN, is men
genoodzaakt om de dosis van de chemotherapie te verminderen of de behandeling
tijdelijk te onderbreken, hetgeen kan leiden tot een slechtere prognose. Een
andere zeer ingrijpende nevenwerking van chemotherapie, is haarverlies. De
incidentie van chemotherapie-geïnduceerde alopecia (CIA) is 65%. CIA is een
nevenwerking die voor veel psychologische problemen zorgt bij de patiënten,
hetgeen hun kwaliteit van leven aanzienlijk beïnvloedt. Ongeveer 8% van de
patiënten weigert een chemotherapie behandeling omwille van het voorspelde
haarverlies. CIA is omkeerbaar, maar vereist een periode van enkele maanden.
In extreme gevallen is het haarverlies permanent. Momenteel is het gebruik van
hoofdhuidkoeling de enige techniek die er gebruikt wordt om het haarverlies in te
perken. Echter, deze behandeling is niet bij iedere patiënt succesvol en brengt
daarnaast ook nog heel wat andere bijwerkingen met zich mee.
Het gebruik van fotobiomodulatie (PBM) therapie heeft sinds de laatste 40 jaar
stilaan zijn plaats gevonden in de algemene geneeskunde (dermatologie,
fysiotherapie tandheelkunde, neurologie,..). Dit is een niet-invasieve therapie
gebaseerd op het gebruik van zichtbaar en/of (nabij-)infrarood licht geproduceerd
door een light-emitting diode (LED) of laser bron. PBM wordt toegepast om de
wondheling te versnellen en om ontsteking, pijn en zwelling te verminderen. Sinds
de laatste 20 jaar groeit het wetenschappelijk bewijs dat deze vernieuwende
behandelingsmethode ook grote voordelen kan opleveren voor kankerpatiënten. Het doel van dit project was het onderzoeken of PBM gebruikt kan worden voor
de preventie en behandeling van CIPN en CIA en zo de levenskwaliteit voor deze
patiënten kan verhogen.
In Hoofdstuk 1 is een literatuuronderzoek uitgevoerd om de toepassing van PBM
voor de behandeling van CIPN te onderzoeken. Slechts één in vivo experiment en
twee klinische studies zijn in het verleden uitgevoerd. Om de kennis rond PBM en
CIPN te vergroten is in Hoofdstuk 2 een gerandomiseerde placebogecontroleerde studie opgezet. Hier werd aangetoond dat PBM het ontstaan van
CIPN bij borstkankerpatiënten gedeeltelijk kan voorkomen. Patiënten die aan de
PBM-groep waren toegewezen hadden een betere levenskwaliteit en sneller
herstel in vergelijking met de controlegroep. Aangezien PBM invloed heeft op de
cel proliferatie, werd de veiligheid van deze behandeling bij kankerpatiënten
onderzocht in Hoofdstuk 3. Na een gemiddelde follow-up periode van 36.3
maanden kunnen we concluderen dat PBM voor de preventie en behandeling van
CIPN geen invloed heeft op het ontwikkelen van uitzaaiingen, metastase of
overleving van patiënten met borstkanker. Om het gebruiksgemak en de dosering
van PBM voor de behandeling van CIPN verder te optimaliseren, werd een
gerandomiseerde gecontroleerde studie opgezet en beschreven in Hoofdstuk 4.
Hier toonden we aan dat met een vereenvoudigd studieprotocol, PBM nog steeds
in staat is symptomen geassocieerd met CIPN te verminderen. De beste resultaten
werden verkregen bij een energiedosering van 8 J/cm². In Hoofdstuk 5 werd
dieper ingegaan op het behandeling van CIA met behulp van PBM. Hier hebben
we waargenomen dat PBM de haargroei na chemotherapie met één maand kan
versnellen hetgeen ook resulteert in een betere levenskwaliteit.
Dit proefschrift geeft meer inzicht in de behandeling van twee destructieve, door
chemotherapie veroorzaakte nevenwerkingen. Deze resultaten vormen de basis
voor toekomstige studies die beogen de kwaliteit van leven tijdens en na een
kankerbehandeling te verbeteren. In 2020, 19 292 789 new diagnoses of cancer were registered worldwide. Almost 60% of these patients were treated with chemotherapy. The goal of chemotherapy is to inhibit cell proliferation and thereby avoid invasion and metastasis. However, chemotherapy does not only affect cancer cells; it also affects, to a greater or lesser degree, all other cells in the body, causing side effects. Since the number of cancer survivors rises, more attention is being paid to those long-term unwanted side effects and the impact they can have on the patient's quality of life (QoL). Chemotherapy-induced peripheral neuropathy (CIPN) is characterized by symptoms such as paresthesia, pain, and a burning sensation in the distal limbs affecting almost 70% of patients receiving neurotoxic chemotherapy. Currently, no preventive measures and limited treatment options can be recommended. A reduction in the chemotherapy dosage, substitutions with other types of chemotherapy, or withdrawal of the chemotherapy is therefore regarded as the most effective method in managing CIPN despite the negative impact it can have on the patients' prognosis. Another common side effect of chemotherapy is hair loss. The incidence of chemotherapy-induced alopecia (CIA) lies around 65%. CIA represents one of the major unmet challenges in clinical oncology and causes psychological distress. For 8% of the patients, the emotional trauma may be so severe that the patient will reject chemotherapy. Hair loss is reversible but requires a period of several months, and in some cases, permanent CIA is reported. Up to now, the only available preventive measure is based on scalp cooling. Nevertheless, this treatment has a highly variable success rate and brings along certain complications. An emerging, preventive and therapeutic tool in the supportive care of cancer patients is photobiomodulation (PBM) therapy. PBM uses visible and/or (near)- infrared light at a low power produced by laser diodes or light-emitting diodes (LED) to stimulate tissue repair and reduce inflammation and pain. PBM has proven its efficiency in general medicine for more than 40 years (dermatology, physiotherapy, neurology, and dentistry). Over the last 20 years, PBM is becoming a new treatment modality in the supportive care of cancer patients. The aim of this project was to investigate the effectiveness of PBM for the prevention and management of CIPN and CIA and the impact of this light therapy on the patients' QoL and OS. In Chapter 1, a literature search was performed to explore the usability of PBM for the management of CIPN. Here we learned that data regarding the management of CIPN with PBM is rather limited. Only one in vivo experiment and two clinical trials could be identified. To increase this knowledge, a randomized placebo-controlled trial was set up in Chapter 2. We proved that PBM is able to prevent the development of CIPN in breast cancer patients partially. Furthermore, patients allocated to the PBM group experienced a better QoL and faster recovery compared to the control. Because of the proliferative nature of this therapy, the safety of PBM to potential circulating cancer cells was examined in Chapter 3. With a mean follow-up period of 36.3 months, current data suggest that PBM for the prevention and management of CIPN does not influence locoregional recurrence, the development of metastases, and the OS of patients with breast cancer. To further optimize the convenience and dosage of PBM for the management of CIPN, a randomized controlled trial was set up and described in Chapter 4. Here we demonstrated that with a simplified study protocol, PBM is still able to reduce symptoms associated with CIPN. Better results were observed with an energy dosage of 8 J/cm² compared to 6 J/cm². Chapter 5 addresses the second focus of this project, the management of CIA with PBM. We observed that PBM is able to accelerate hair regrowth post-chemotherapy by one month in breast cancer patients. This outcome was supported by an improvement in the patients’ QoL and body image. The results of this thesis provide further insight into the management of two devastating chemotherapy-induced side effects. These data form a basis for future studies that aim to improve the overall QoL of patients during and after cancer therapy. |
