Title: | Autism and transportation: exploring experiences, viewpoints, and participation among autistic individuals and their caretakers | Authors: | DIRIX, Hélène | Advisors: | Brijs, Kris Ross, Veerle |
in kaart gebracht.
uitdagend ervaarden, waarbij ongeveer vijfendertig procent professionele hulp
tien hoofdthema's, onderverdeeld in vier groepen.
vervoerswijzen gebruiken (bijv. trein, tram) of specifieke situaties vermijden (bijv.
Autism is a commonly neurodevelopmental condition diagnosed in approximately 1 in 100 individuals worldwide. It is characterized by challenges in social interaction, communication, and repetitive and restrictive behaviors. These symptoms exist along a spectrum, meaning that each person with autism displays a unique set of symptoms with varying degrees of intensity, contributing to the diversity within the condition. Due to these distinctive characteristics, autistic individuals encounter difficulties from childhood on in various essential aspects of daily life, such as self-care, social interactions, employment, and transportation. However, the availability of safe, accessible, and comfortable transportation profoundly impacts an individual's overall well-being, underscoring the significance of ensuring universal access. Through the chapters in this thesis, we aimed to contribute significantly towards a more comprehensive understanding of the relationship between autism and transportation within various target populations, encompassing autistic children and young adolescents, caregivers of autistic individuals, and autistic adults. When addressing societal challenges and striving to enhance overall well-being, two crucial factors should be considered: the behavior of a specific group and their environment. In this dissertation, that societal issue is defined as the inaccessibility, lack of safety, and discomfort experienced by autistic individuals in transportation. To effect meaningful change and provide tailored support that aligns with individuals' unique needs, it is essential to examine the underlying psychosocial factors that contribute to this problem. This approach allows us to tailor interventions and policies that encourage desired behaviors while accommodating individual and environmental preferences. Furthermore, it establishes a groundwork for identifying areas warranting future research. This dissertation employs the Capability, Opportunity, Motivation - Behavior (COM-B) model to explore and identify the underlying determinants influencing accessible, safe, and comfortable transportation for autistic individuals. According to the COM-B model, behavior arises from the interplay of three fundamental components: Capability, Opportunity, and Motivation. By analyzing these determinants and their interactions, we can map the dynamics between various concepts and facilitate the adoption of desired behaviors. In essence, to ensure that autistic individuals can access safe, convenient, and comfortable transportation, it is imperative to identify the underlying determinants that impact this situation both positively and negatively. This insight will inform future research and guide the development of interventions to foster improvements. This thesis comprises five empirical chapters that follow a person's life course. The first two chapters focus on autistic young adolescents and their caregivers. The subsequent chapter shifts to the period of licensure, where individuals aim to obtain driver's licenses. Finally, the experiences of autistic adults while driving and using public bus transportation are explored. In Chapter 1, we conducted a quantitative exploratory study to assess the hazard perception skills of male autistic young adolescents in Jakarta, Indonesia. The study aimed to examine to which extent autistic young adolescents differed from their non-autistic peers regarding hazard recognition rates across various hazard categories, reaction times, hazard danger level estimations, and travel characteristics. Specifically, we recruited twenty autistic young adolescents and thirty-four non-autistic peers who completed an online questionnaire consisting of three sections: demographic data, travel characteristics, and a hazard perception test. The hazard perception test included twenty photos, with five photos in each of the following categories: Behavioral Prediction Hazards (BP), Environmental Prediction Hazards (EP), Dividing and Focusing Attention Hazards (DF), and photos without hazards. Participants were tasked with identifying where hazardous situations could occur in these photos and rating the perceived danger level of each situation. Reaction times and the number of correct answers were recorded. Findings revealed differences in travel characteristics between the two groups; for instance, autistic participants walked more frequently for discretionary and leisure activities than their non-autistic counterparts. Regarding test performance, autistic young adolescents had a significantly lower number of correct answers in the BP and DF categories. Their reaction times were also worse for the BP, EP, and DF categories. Furthermore, no significant differences were observed in the no-hazard category. Moreover, no substantial differences were found in danger level estimations. The main underlying determinants that could be linked to this study, which influenced the hazard perception skills of autistic young adolescents, were knowledge, cognitive skills, attention, and social support. In Chapter 2, a quantitative exploratory study that focused on mothers' perceptions of hazard perception skills in male autistic young adolescents residing in Jakarta, Indonesia, was conducted. Building upon the research conducted in Chapter 1, this study specifically delved into how mothers perceived their autistic child's hazard perception skills concerning their performance and comparison with a control group. Additionally, it explored the relationship between parents' perceptions and their children's actual hazard perception skills. Twenty mothers participated in this study and were presented with the same set of pictures used in the hazard perception test their children had taken. Following this, a questionnaire was administered to assess the mothers' perceptions of their child's performance on the hazard perception test and their opinions regarding their child's current and future hazard perception skills. The results revealed that sixtyfive percent of all parents expressed concerns about their child's current hazard perception skills. In comparison, only thirty-five percent expressed concerns about their child's future skills. Additionally, eighty-five percent believed that hazard perception skills would be important in the future, compared to seventy percent who believed they were currently important. Regarding parents' prediction accuracy, no significant differences were found when comparing parents’ predictions with their children’s number of correct answers and their children's performance relative to the control group. However, parents consistently underestimated their children's reaction times compared to the nonautistic control group. Several factors were associated with a decreased likelihood of parents overestimating their children's hazard perception abilities. These factors included the number of car use days per week, infrequent car use, motorcycle use for discretionary activities, the number of correct answers on the test, and reaction time. Conversely, children's increased car use for discretionary activities and having an older non-autistic sibling were linked to decreased prediction accuracy. The main underlying determinants that could be linked to this study, which influenced the mothers’ perception, were skills assessment, environmental resources, social support, and perceived competence. In Chapter 3, a quantitative exploratory study to investigate the experiences of autistic drivers in Belgium with the licensing process, focusing on the theoretical and practical examination moments, was conducted. Participants completed a questionnaire to provide insights into their experiences and preparations for these examinations. The findings revealed that approximately twenty-five percent of participants found the theory exam challenging, with about thirty-five percent seeking professional assistance for the preparation. For the practical exam, these percentages increased to approximately fifty percent and sixty percent, respectively. Notably, time constraints and question phrasing were commonly cited issues for the theoretical exam. Roughly half of the participants faced communication difficulties with the examiners, while more than sixty percent experienced unpredictability during the practical exam. It was found that the more a participant had to take an exam (both theoretical and practical), the more difficult they perceived the exam. Similar results were observed regarding the level of difficulty and experiences during preparation. Additionally, those who felt time pressure during the theoretical test were more likely to find the test itself challenging, and those who experienced time pressure also experienced more difficulties with the question formulation during the theoretical exam. However, no significant relationships were found between the frequently reported problems during the driving exam and factors such as assistance during exam preparation, experiences during preparation, perceived difficulty of the practical exam, or the number of practical exam retakes. The main underlying determinants that could be linked to this study, which influenced the licensure experiences, were interpersonal skills, person x environment interaction, barriers and facilitators, social support, and stress. In Chapter 4, a qualitative study to explore how autistic drivers perceive and navigate elements of the roadway environment and infrastructure was conducted. Twelve autistic drivers participated in semi-structured interviews with a photo guide to support and clarify questions. The interviews were analyzed using the phenomenological hermeneutical method to investigate the participants' subjective experiences and perceptions. The study identified ten main themes, categorized into four groups. The first group, 'Positive and Negative Feelings While Driving' (Themes 1-3), uncovered elements of the roadway environment and infrastructure that evoked positive feelings, such as unambiguous traffic signs and separate bike paths, as well as negative emotions, including unclear signage and aggressive driving behavior. The second group, 'Situations That Negatively Affect Driving Experiences' (Themes 4-6), explored factors that made driving more complex (e.g., neon signs, the constant noise a car makes, etc.), the inefficient application of traffic rules in difficult situations (e.g., driving over a full white line, etc.), and rush and chaos (e.g., driving in a city center, many traffic signs, etc.). The third group, 'Experience and Automatization' (Theme 7), highlighted how participants' driving experience helped them handle difficult and novel situations despite occasional challenges. The fourth group, 'Coping Strategies to Minimize Negative Experiences While Driving' (Themes 8-10), revealed various strategies, such as adjusting behavior (e.g., turning off the radio to reduce incoming stimuli, etc.), using alternative transportation modes (e.g., train, tram, etc.), or avoiding specific situations (e.g., not driving during rush hour, etc.). The main underlying determinants that could be linked to this study, which influenced the appraisal of roadway environment and infrastructure in autistic drivers, were coping strategies, environmental stressors, person x environment interaction, action planning, positive affect, and negative affect. In Chapter 5, a qualitative study to explore the experiences of autistic individuals when using public bus transportation was conducted. This study aimed to give autistic individuals the opportunity to share their experiences and concerns regarding public bus transportation by inquiring about their lived experiences and real-life encounters during their journeys. Semi-structured interviews, accompanied by a photo guide, were conducted with seventeen autistic individuals who regularly used public bus transportation. The data collected was analyzed using interpretative phenomenological analysis. Three main themes emerged from this study, shedding light on factors crucial for enhancing the comfort of bus travel for autistic individuals. It became apparent that participants often experienced increased stress, anxiety, and frustration when these aspects were not adequately addressed during their journeys. The first theme, 'Creating Predictability,' encompassed three underlying subthemes contributing to predictability: (1) overview throughout the journey (i.e., having a mental overview throughout the trip to enhance predictability), (2) consistency (i.e., reliability and uniformity of services, schedules, and routes provided to passengers), and (3) accuracy of information (i.e., the correctness and precision of details and updates conveyed to passengers regarding schedules, routes, and service modifications). The second theme, 'Limiting Sensory Stimuli,' highlighted the challenges participants faced in processing the various stimuli encountered during bus journeys, often resulting in fatigue. Coping strategies, such as the use of noise-canceling headphones, were identified as effective methods for alleviating these challenges. The third theme, 'Open and Accessible Communication,' underscored participants' need for an environment that promotes comfortable interaction and provides easy access to assistance and information. Unfortunately, this need was frequently unmet, leading to heightened stress and anxiety when seeking help. The main underlying determinants that could be linked to this study which could be linked to the autistic persons' experiences while using public bus transportation were cognitive overload, coping strategies, barriers and facilitators, environmental stressors, person x environment interaction, positive affect, and negative affect. Several clinical and practical implications were derived from the studies presented in this thesis. These include the development of a hazard perception course designed for autistic children and their parents. Additionally, there is a need to offer a special practical driving exam that accommodates the unique needs of autistic individuals. Establishing peer support groups where autistic individuals can share their experiences and coping strategies is another valuable initiative. In terms of future research directions, it is advisable to explore the generalizability of the findings across different regions and age groups. Moreover, conducting research that delves into the lived experiences of autistic individuals can provide further insights into the experiences of autistic individuals with regard to accessible, safe, and comfortable transportation. |
