Abstract: The replacement of a painful thumb carpometacarpal joint with a ball-and-socket prosthesis was introduced over 50 years ago. Continuous improvements in implant design and a surgical technique have made it a successful procedure. However, complications remain possible. This review discusses key aspects for optimising indications, surgical techniques, and treatment outcomes, as well as the management of complications when they occur. Options for revision surgery are also explored. Total arthroplasty of the thumb carpometacarpal joint represents a valuable treatment option. Knowledge of correct indications, surgical techniques, potential pitfalls, and alternative treatments is essential.
