| dc.description.abstract | Anti-CD20-behandelingen zijn zeer effectief bij het onderdrukken van focale ontstekingen bij multiple sclerose (MS). Ze bieden echter slechts beperkte werkzaamheid tegen progressie onafhankelijk van terugvalactiviteit (PIRA) vanwege farmacokinetische barrières in het centrale zenuwstelsel (CZS). Cladribine, een klein molecuul dat de bloed-hersenbarrière effectiever passeert, biedt een goed alternatief. Niettemin blijft de optimale duur van het behandelingsinterval na het staken van de anti-CD20-therapie en het starten van cladribine een onderwerp van discussie vanwege de risico's van cumulatieve immunosuppressie of hernieuwde ziekteactiviteit. Deze single-center, retrospectieve, real-worldstudie evalueerde de effectiviteit van cladribine bij 43 personen met de diagnose MS die waren overgestapt van anti-CD20-therapie (voornamelijk ocrelizumab), voornamelijk vanwege functionele achteruitgang (69,8%). Er werden geen statistisch significante verschillen waargenomen tussen de behandelingsintervalgroepen wat betreft NEDA-3, bevestigde verslechtering van de invaliditeit (CDW), activiteit op magnetische resonantiebeeldvorming (MRI) of terugvallen. De gegevens toonden wel een stabiliserend effect van de overbruggingsbehandeling op de ziekteactiviteit aan. Dit ondersteunt de flexibiliteit en veiligheid van deze behandelingsoverstap in de klinische praktijk. | |
